Need to report a failing street light, traffic signal, or other traffic-related issue?

Problems can be reported 24 hours a day, 7 days a week to the City using the Request for Service form via AviWise, Avondale’s virtual service request system.

To ensure the best possible service, be prepared to provide the following information:

• The closest street address to the streetlight outage.  If unknown, please provide major cross streets.
• Streetlight pole number, which is located 6 - 10 feet up from the bottom of the pole.
• Name and number so the technician can contact them with any questions or for further clarification.
