Voucher Ride Taxi Subsidy Program is for Physician Ordered Repetitive Treatments and Therapies

The Taxi Subsidy Program is designed to assist people making repetitive visits for medical treatments and therapies. This may be the answer you have been looking for if you or someone you know receives such treatments and needs transportation assistance to get to and from a treatment facility.

Who is eligible?

Avondale and Glendale residents who receive qualifying physician ordered repetitive treatments and therapies such as dialysis, cancer treatments, and therapy following a stroke are eligible for this service.

Where do my trips begin and end?

Vouchers are addressed to two locations. The first is your home and the second is the medical treatment centers with the city limits.

How are my rides arranged?

Participants in the program call a taxi or van company of their choice from a list of participating companies and arrange their own rides. Participants are free to change taxi companies at any time. Wheelchair accessible service is available.

How much do I pay?

Avondale and Glendale pays 75% of a one-way taxi fare up to $15.00 per trip. You pay 25% of the fare. The City will also pay 15% for gratuity (up to $2.50 per trip).

How do I get the vouchers?

For dialysis, the social worker at your center will supply vouchers. For other medical trips, vouchers will be mailed directly to participants. To sign up for this program, visit your social worker or call Voucher Ride at (623) 414-4765.

How do I use the vouchers?

At the time of each trip, participants present a voucher to the driver. Each voucher indicates that it pays 75% of the fare up to $15.00. The participant is responsible for 25% of the charges plus any charges over the $15.00 limit. The vouchers are signed by both the participant and the taxi driver who submits the voucher to the taxi company.
