Notice Regarding Fire & Medical Records

The City of Avondale does not bill for Fire & Medical services rendered; therefore, we do not have patient billing records.

Notice Pursuant to A.R.S. §39-171: 

Entities subject to the public records law must make available on their websites the name, telephone number, and email address of an employee or department that is authorized and able to provide the information requested or able to forward the request to an employee or department that is authorized and able to provide the information requested.

 

Pursuant to this section, all public records requests to the City of Avondale should be directed to:

Response Times: 

Public records requests are processed by the City Clerk's Office, with the exception of Police and Court records. Authorization from the City Attorney may be needed before records are released. We strive to respond to all requests within 72 hours. However, immediate availability of the requested records or complexity of the request may cause a delay.

Public Records Request Fees: 

If a Public Records Request Fee is charged, you will be notified.
