When applying for a U.S. passport, always go to the official government website that ends in .gov (NOT .com or .org).

Passport Services Available by Appointment Only

REQUIREMENTS FOR PASSPORTS
All applicants must be present, including children.

1. Complete all necessary forms.

Forms must be printed single sided and should be completed in black ink.

Application for New Passport and Child Renewals (Form DS-11)
Statement of Consent for a Child Passport (DS-3053)
Additional forms (U.S. Dept. of State website)

2. Provide proof of citizenship (original and a copy).

Certified birth certificate, passport, or naturalization certificate.

For minors 15 and under, child and both parents must be present.
- If minor child is presenting a passport as citizenship, a photo copy of birth certificate is also required.
For minors 16-17, child and one parent must be present.

3. Provide proof of identity (original and a copy).

Valid government issued picture ID.

For minors, parents must present valid ID.

4. Provide a passport photo.

Please bring at least one passport photo with you.

5. Pay the City of Avondale (card only)

The City of Avondale execution fee must be paid by credit or debit card only (see fees posted below).

Beginning June 1, 2026, a convenience fee will be applied to payments made by credit card. In-person credit card payments will be assessed 3.5% of the transaction amount. There is no fee for passport fees made by debit cards (with PIN).

6. Pay the U.S. Dept. of State (check or money order).

The U.S. Department of State passport fees must be paid by check or money order only (see fees posted below). Each applicant must have their own check or money order.
