Passport services will be CLOSED on the following dates:

  • May 27, 2019 
  • June 3rd, 2019
  • June 10th thru June 13th, 2019

Office hours:

Monday - Thursday, 7 a.m. to 2 p.m.   We are CLOSED on Friday's. 

We accept appointments (during limited hours) and walk-ins. 

Appointments can ONLY be schedule between the hours of 8am - 12pm.   Appointments will be seen first during this time frame (8 - 12). Walk-ins are seen from 7am - 2pm, however you may experience a long wait time from 8am - 12pm if you do not schedule an appointment.   To schedule an appointment, please call 623-333-1200.   

NOTE:  If you arrive for a scheduled appointment and you are not prepared with proper documents and forms of payment, you appointment will be cancelled.  See below for required documentation and payment information. 

To apply for a NEW PASSPORT, you will need:

APPLICANTS 18 AND OLDER:

  • Applicant must be present
  • Application DS-11.  You may obtain one from Avondale City Hall or by visiting www.travel.state.gov
  • A certified copy of your birth certificate, original certificate of naturalization or certificate of birth abroad. You will ALSO need a copy of this document. (a self-serve copy machine is available at our facility; copies are 25 cents).
    • Hospital record of birth is not accepted
    • The long form of birth certificate is required.  Applicants who were born in states that issue short forms or abstract versions will need to obtain the long form
    • Original documents will be submitted with the application, but will be returned by the US Department of State
    • How to obtain a certified copy of your birth certificate
  • Valid government issued picture ID. You will ALSO need a copy of the front and back of this ID. 
  • Passport photo - this service is available for $15.00.  
  • ONE check (personal or cashier) or ONE money orders for the passport fee (fees listed below).   Execution and photo fees can be paid with any form of payment. 

APPLICANTS 16 - 17 YEARS OF AGE:

  • Applicant must be present
  • One parent must be present 
  • Application DS-11.  You may obtain one from Avondale City Hall or by visiting www.travel.state.gov
  • A certified copy of your birth certificate, original certificate of naturalization or certificate of birth abroad. You will ALSO need a copy of this document. (a self-serve copy machine is available at our facility; copies are 25 cents).

APPLICANTS 15 YEARS AND YOUNGER:

  • Applicant must be present
  • BOTH parents must be present if both parents are listed on the birth certificate. 
  • Application DS-11.  You may obtain one from Avondale City Hall or by visiting www.travel.state.gov
  • A certified copy of your birth certificate, original certificate of naturalization or certificate of birth abroad. You will ALSO need a copy of this document. (a self-serve copy machine is available at our facility; copies are 25 cents).

 RENEWALS :

You can ONLY renew a passport if it was valid for 10 years and has not been expired for more than 5 years.  Children's passports CANNOT be renewed.  Children must apply for a NEW passport until they acquire an 'adult' (10 year) passport. 

Passport renewals are processed by mail only and should be submitted to the US Department of State directly by the applicant.  You may obtain a passport renewal application (DS-82) by visiting Avondale City Hall or at www.travel.state.gov
